Why Does My Dry Eye Persist Even Though I Use Eye Drops?

Yazar: Op. Dr. Gökay Günay

Many people with dry-eye symptoms notice that their complaints continue despite using artificial tears.

Common complaints include:

  • A feeling of dryness that returns during the day
  • Burning and stinging
  • Blurred vision
  • Discomfort while using a computer
  • Eye fatigue that increases in the evening

In this situation, many patients ask:

“I use eye drops, so why am I still uncomfortable?”

Often, the reason is not that the drops are inadequate, but that the underlying mechanism of the dry eye has not been fully identified.

Dry Eye Is Not a Single Disease

When people hear “dry eye”, many think only of insufficient tear production.

However, dry eye can arise for different reasons.

In some people tear production is reduced, while in others the amount of tears may be adequate but their quality is impaired.

For some people, the main problem is that the oil glands in the eyelids are not functioning properly.

For this reason, the same treatment approach may not work for every patient with dry eye.

Why Might Artificial Tears Not Be Enough?

Artificial tears provide significant relief for many patients.

However, for some people their effect may be short-lived.

This is often because the drops relieve symptoms without changing the underlying cause.

For this reason, symptoms may return when the effect of the drops wears off or when they are stopped.

Why Are the Eyelid Glands So Important?

Not only the amount of tears, but also their quality is important.

Special oil glands in the eyelids produce a protective layer that slows tear evaporation.

When this system does not work well enough, tears may evaporate more quickly.

As a result, the person may experience persistent dryness.

Can Computer Use Make the Problem Worse?

Yes.

Blinking frequency decreases, particularly in people who spend long hours in front of a screen.

This may cause the ocular surface to dry more quickly.

For some people, this explains why the same symptoms return by the end of the day despite using drops.

Can More Advanced Treatments Be Needed?

For some patients, artificial tears alone may be sufficient.

However, others may need more comprehensive treatment directed at the cause of the dry eye.

Particularly when the eyelid glands are affected, treatment options aimed at supporting ocular surface and eyelid function, such as IPL (intense pulsed light), may also be considered.

What matters is not applying the same treatment to every patient, but correctly identifying the source of the problem.

Is Dry Eye Only a Comfort Problem?

No.

In some people, dry eye may cause not only burning and stinging, but also:

  • Blurred vision
  • Fluctuating vision
  • Reduced night vision
  • Reduced visual performance

These visual effects may also occur.

For this reason, the aim of modern dry-eye treatment is not only to provide relief, but also to improve ocular surface health and quality of vision.

Conclusion

If dry-eye symptoms continue despite using eye drops, the underlying cause of the dry eye may not yet have been fully targeted.

Successful treatment depends not only on suppressing symptoms, but on understanding the cause of dry eye and creating an individualised treatment plan.
